Agomelatine: An Unusual Mechanism, Sleep, and Mandatory Attention to the Liver

Agomelatine is often described with the appealing phrase “restores circadian rhythms.” Its mechanism really is different from SSRIs: it acts at melatonin MT1/MT2 receptors and blocks 5-HT2C receptors. But behind the sleep narrative lies a very concrete safety issue: the liver.
What is driving this
In the European Union, agomelatine is approved for major depressive episodes in adults. It is not simply “stronger melatonin” and it is not a universal anti-anxiety treatment.
In a large network meta-analysis, agomelatine had favorable treatment acceptability, although evidence quality varied across comparisons. A newer review of antidepressants and sleep also found improvements in subjective sleep, while objective sleep results were less consistently positive.
EMA requires regular liver-function monitoring. Tests are performed before treatment and at specified points after starting or changing the dose. The drug is contraindicated in active liver disease and significant transaminase elevation, and safety measures were strengthened after reports of hepatic injury.
This is a useful example of how popular descriptions can omit the most important medical detail. A patient reads “better sleep, fewer sexual side effects” and wonders why a psychiatrist is ordering blood tests. Those tests are part of safe use, not administrative fuss.
Agomelatine is an interesting antidepressant with an unusual mechanism. Its uniqueness does not cancel the basic rule that a benefit is meaningful only when the safety requirements are followed.

Agomelatine can sound attractive because of its unusual mechanism and sleep-related profile, yet it also requires attention to liver function. This contrasts with the myth that “not an SSRI” automatically means simpler or safer. Different mechanisms bring different potential benefits and monitoring needs.

After starting treatment, track changes in sleep, anxiety, mood, functioning, and side effects over time. Sudden deterioration, suicidal thoughts, marked agitation, signs of mania, an allergic reaction, or another acute symptom require prompt medical contact or emergency care. Do not change the dose or discontinue a prescription medicine on the basis of an article.
